Welcome.
You can do either (or both). Obviously the PAL tuner is for analog VHF/UHF broadcasts. The paytv (or FTA) box plugs in to the AV inputs on the back of the TiVo without any hardware modifications. You can add a second set of audio inputs so that you can plug one box into the svideo input, and one into the composite input, so you can have FTA + paytv.
Note that what ever configuration you use, the TiVo can only record from one at a time.
Of course you will need to reimage the TiVo HDD with an OzTiVo image CD on your computer.
Where are you located?
Plenty of info on www.oztivo.net

Are you located in Australia or New Zealand?

The most common way of record Free to air Channels these days is to use a Digital Set Top Box instead of having an internal tuner fitted. The currently supported boxes are listed here (http://www.oztivo.net/tivo-bin/irdatabase.cgi) (you will need to register here (http://www.oztivo.net/twiki/bin/view/TWiki/TWikiRegistration) to see it).

As above comments. Yes I can still do PAL tuner installs but in all honesty with the extra channels you get on digital and the cheap prices on set top boxes I really would not bother with an internally fitted analogue tuner.

Please update your profile to reflect roughly where you live. It makes the job of helping answer specific questions a lot easier :)
